Impaired Driving Assessment
DWI/DWAI/DUIs are serious offenses with significant legal, financial, and personal consequences. They require a trained, experienced, and caring professional to work through the process for the protection of the client and the community.
SCREENING / ASSESSMENT MAY BE REQUIRED WHEN:
order alcohol/drug screening/evaluation prior to sentencing
request comprehensive evaluation- Impaired Driving Assessment
refer you for evaluation if they suspect alcohol/drug abuse problem.
recommends you having evaluation done prior your court appearance.
required to complete your probation menu or requested by your probation officer
YOU MUST BRING THE FOLLOWING TO YOUR APPOINTMENT:
o Arrest report and/or ticket with BAC listed on the form
o Court papers (if referred by court)
o Copy of your driving license or other ID
o Abstract of Driving Record (obtained from DMV for $10 fee)
o Name and phone number of 2 Collateral Sources
o Your payment – cash, check or credit card

COURT MANDATED SUBSTANCE ABUSE SCREENING– Two or more approved screening instruments (questionnaires) AND URINE TEST FOR ALCOHOL AND DRUGS are used to determine if you drinking behavior is safe, risky, or harmful. Average time: 2 sessions, 45-60 minutes. Fee for screening and necessary paperwork/reports/letters - $265 for 2 sessions. Urine test is $35. Total-$300
COURT MANDATED SUBSTANCE ABUSE ASSESSMENT – Assessment is conducted if the court ordered you to undergo assessment or if screening indicated that your drinking problems are risky or harmful. This is an in-depth interview that includes psychosocial assessment, mental health history, collateral interview, and alcohol/substance use history and urine test for alcohol and drugs. Average time – 1-2 sessions, 45-60 minutes each. Number of sessions depends on the circumstances of your case. Fee for assessment and necessary paperwork/reports/letters - $265 for 2 sessions. Urine test is $ 35. Total- $300
IMPAIRED DRIVING ASSESSMENT – (DS-449) FORM - This is in-depth interview that includes psychosocial assessment, mental health history, alcohol/substance use history, collateral interview, urine test for drug and alcohol, and the use of two or more approved screening instruments (questionnaires). In order to complete the Impaired Driver Assessment the clinician must receive the following: Abstract of Driving Record, Arrest Report/ Ticket with BAC listed, Name and Phone Number of 2 Collateral Sources, Urine Alcohol and Drug Screen. Average time – 2-3 sessions, 45-60 minutes each. Number of sessions depends on the circumstances of your case. Fee for assessment and necessary paperwork/reports/letters- $265 for 2 sessions. Urine test is $35. Total- $300
